Melyssa Higgins, Ph.D. Scheduling an Evaluation with Dr. Higgins Thank you for considering a psychoeducational evaluation with Dr. Higgins. We look forward to working with you. Before your child is placed on her schedule, we ask that you complete two required forms and return them to our office. The first form provides us with information about how to contact you and the reasons why you are seeking this kind of evaluation (Registration Form). The second form provides you with information about Dr. Higgins’ policies (Preliminary Financial Agreement). You may mail, fax, email, or hand carry them to our office. Once we have received both of your completed forms, we will contact you to discuss dates and times that are currently available on Dr. Higgins’ schedule. If you are considering an application to a private school program designed for highly capable learners and your child needs an intellectual assessment specifically for this purpose, then please contact our office at (206) 361-6884 for information about fees and scheduling. Insurance: As of August 17, 2012 Dr. Higgins will no longer be contracted with any insurance companies and therefore we cannot direct bill your insurance provider. After you have paid for your appointment, we will provide you with a statement that contains the information that your insurance requires from us to process your claim. You will be reimbursed directly from your insurance company if any amount is due to you. Please be aware that many plans do not cover out-of-network mental health services. Dr. Higgins’ evaluation services are coded under mental health procedure codes. It is important that you find out exactly what mental health services your plan covers before you come to your first appointment. Payment: It is Dr. Higgins’ policy to collect payment in full at the beginning of each appointment. Our office gladly accepts cash, check, VISA, and MasterCard.
